Terms and Conditions

My child will wear suitable and appropriate clothing, footwear and equipment.

All players are required to purchase R17 Football Kit and it must be worn to every training session/ R17 event

I understand that participation is of a physical nature. I declare as a condition of participation, that my child has sufficient personal fitness to be able to safely participate and that I am not aware of any illness, injury, disability or condition which may preclude my child from participating and/or cause my child injury, death or loss while participating in R17 Football Academy programs and activities.

I understand that I must disclose any pre-existing medical or other condition that may affect the risk that either my child or any other person will suffer injury, loss or damage and waiver any claim they might have on.

I consent to first aid being administered by a R17 Football Academy staff member who is the holder of a current first aid certificate.

I agree that if my child is injured or ill that R17 Football Academy and its affiliates may at my cost and expense arrange medical treatment and emergency transportation to hospital or a doctor as deemed necessary, by R17 Football Academy and its affiliates. I agree that I am responsible for my child's medical and ambulance costs and insurance.

IN CONSIDERATION OF my child, being allowed to participates in any way in R17 Football Academy related events and activities, and agrees that:

The risk of injury to my child from the activities involved in these programs is significant, including the potential for permanent disability and death, and while particular rules, equipment, and personal discipline may reduce this risk, the risk of serious injury does exist; and,

FOR MYSELF, SPOUSE, AND CHILD, I KNOWINGLY AND FREELY ASSUME ALL SUCH RISKS, both known and unknown, EVEN IF ARISING FROM THE NEGLIGENCE OF THE RELEASES or others, and assume full responsibility for my child's participation; and,

I myself, my spouse, my child, and on behalf of my/our heirs, assigns, personal representatives and next of kin, HEREBY RELEASE AND HOLD HARMLESS R17 Football Academy: its directors, officers, officials, agents, employees, volunteers, other participants, sponsoring agencies, sponsors, advertisers, and if applicable, owners and lessors of premises used to conduct the event ("Releases"), WITH RESPECT TO ANY AND ALL INJURY, DISABILITY, DEATH, or loss or damage to person or property incident to my child's involvement or participation in these programs, WHETHER ARISING FROM THE NEGLIGENCE OF THE RELEASEES OR OTHERWISE, to the fullest extent permitted by law.

I agree for my child's personal details to be entered into a database held by R17 Football Academy which will be used for the administration of programs and activities, and may also be used for future marketing and promotion of R17 Football Academy programs and activities.

I give consent for R17 Football Academy staff members to take photographs or videos of the classes and events where my child might attend, for marketing and/or publicity use. Photographs/videos taken by parents/guardians may only be used for personal use.  

Please note that refunds are not available. Full commitment to the program is required; if you are unable to attend a session, it will be considered a forfeiture of that session.

 I HAVE READ THIS RELEASE OF LIABILITY AND ASSUMPTION OF RISK AGREEMENT, FULLY UNDERSTAND ITS TERMS, UNDERSTAND THAT I HAVE GIVEN UP SUBSTANTIAL RIGHTS BY SIGNING IT, AND SIGN IT FREELY AND VOLUNTARILY WITHOUT ANY INDUCEMENT. UNDERSTANDING OR RISK: I understand the seriousness of the risks involved in participating in this program, my personal responsibilities for adhering to rules and regulation, and accept them as a participant.
